CCA Cares form for Students

This page serves as a resource for CCA Students. Students are the cornerstone of our CCA Community. As the eyes and ears of the College community, it is everyone's responsibility to assist those students who exhibit concerning behavior, or are in need of individualized resources/support.

The Dean of Students Office encourages you to utilize the CCA Cares reporting process in order to do your part to actively participate in efforts to keep the community safe and functional. The success of this process is dependent upon community commitment to reporting concerns.

The Dean of Students accepts CCA Cares forms from any person who is concerned about the health or safety of a CCA student, employee, visitor, or the campus community at large. Once this form is submitted, the Dean of Students Office will review the information and take appropriate action, which may or may not include contacting you, the reported/referred person, and any others you have identified that may have relevant information or who may have been impacted by the reported/referred person. If you wish to make an anonymous report, the team will evaluate the report and determine what follow up (if any) is necessary to address the reported concerns.

While anonymous reports are accepted, you are encouraged to provide your name and contact information in order to allow us to most effectively and efficiently address the content of which you are reporting. We must have the name of the person you are concerned about in order to follow-up and provide appropriate assistance.


ATTENTION

DO NOT report emergencies via the CCA Cares form. If this is an emergency call 9-1-1 and then call CCA Public Safety at 415.703.9510. If this is a psychological emergency, please contact CCA’s Mental Health Crisis Hotline at 415.551.9344. After submitting the form, you will receive a confirmation email. This information will be reviewed within 3 business days. You can expect a delayed response during college closures.
