CCA Cares form for Faculty & Staff

This page serves as a resource for our Faculty & Staff members. Faculty & Staff members are an integral part of our CCA Community as they support our students. You'll find a link to the CCA Cares form submission page as well as several resources to help coach you through the process of engaging and encouraging students through their journey here at CCA.

CCA Faculty & Staff are required to report information that may or potentially violate campus policies (Student Code of Conduct, Sexual Misconduct policy, Discrimination, Unlawful Harassment & Retaliation). By filing out a CCA Cares form for Faculty & Staff, they fulfill this obligation. Additionally, students may disclose other information that may rise to a level of needing supplemental intervention. By reporting information via the CCA Cares form this does not automatically mean the student or reported party will be in trouble. This means the reported information will be reviewed and assessed by subject matter experts to determine the best next steps for all parties involved to move forward.


ATTENTION

DO NOT report emergencies via the CCA Cares form. If this is an emergency call 9-1-1 and then call CCA Public Safety at 415.703.9510. If this is a psychological emergency, please contact CCA’s Mental Health Crisis Hotline at 415.551.9344. After submitting the form, you will receive a confirmation email. This information will be reviewed within 3 business days. You can expect a delayed response during college closures.

Once this form is submitted, the Dean of Students Office will review the information and take appropriate action, which may or may not include contacting you, the reported/referred person, and any others you have identified that may have relevant information or who may have been impacted by the reported/referred person.
