CCA Cares form for Community Members

This page serves as a resource for community members. Community members play a huge role in the success of our students whether they are mentors, family members, friends, and/or employers. Our students often confide in community members to express their achievements as well as their shortcomings. This webpage serves as resource for community members as they continue to support and encourage their Student on their journey at CCA.

By reporting information via the CCA Cares form this does not automatically mean the student or reported party will be in trouble. This means the reported information will be reviewed and assessed by subject matter experts to determine the best next steps for all parties involved to move forward.

Once this form is submitted, the Dean of Students Office will review the information and take appropriate action, which may or may not include contacting you, the reported/referred person, and any others you have identified that may have relevant information or who may have been impacted by the reported/referred person. If you wish to make an anonymous report, the team will evaluate the report and determine what follow up (if any) is necessary to address the reported concerns.

While anonymous reports are accepted, you are encouraged to provide your name and contact information in order to allow us to most effectively and efficiently address the content of which you are reporting. We must have the name of the person you are concerned about in order to follow-up and provide appropriate assistance.


ATTENTION

DO NOT report emergencies via the CCA Cares form. If this is an emergency call 9-1-1 and then call CCA Public Safety at 415.703.9510. If this is a psychological emergency, please contact CCA’s Mental Health Crisis Hotline at 415.551.9344. After submitting the form, you will receive a confirmation email. This information will be reviewed within 3 business days. You can expect a delayed response during college closures.
