New Employee Technology Requisition
This form is now automatically requested when submitting a new staff hire in Workday. Use this form for any non-standard technology requests.

Salesforce Access Request Form
For new hires, this form is now incorporated into the Technology Requisition form in Workday. Use this form to request access to Salesforce for an existing employee. The form should be filled out electronically, digitally signed, and emailed to helpdesk@cca.edu.

Contractor/Consultant Data Access Request Form
Use this form to request access to various CCA systems for a contractor or consultant. The form should be filled out electronically, then printed, signed, and interofficed to the Help Desk / ETS-Oakland or emailed to helpdesk@cca.edu.

Employee Termination Technology Checklist
This form is now automatically sent to the manager to complete when a staff member is terminated in Workday. Use this form for non-standard system access termination requests. This form should be filled out electronically, electronically signed, and then emailed to helpdesk@cca.edu. For immediate access revocation, please contact the Help Desk at 510.594.5010.

Data Security and Confidentiality Agreement
This form is automatically sent to all new hires in Workday for electronic signature. It's provided here as a reference for CCA policies surrounding Data Security and Confidentiality.

Technology Equipment Agreement
This form is required for all staff who are issued laptops or other CCA technology equipment. Please also see instructions for completing the form electronically.
